Nominate an Award Recipient

Introduction

The Parkinson’s Industry Awards recognise individuals and organisations making an exceptional contribution to Parkinson’s care, research, innovation and community support.

The Awards will be presented at the Parkinson’s Gala Dinner & Industry Awards on Saturday, 10 October 2026, at Maroochy RSL.

Before beginning, please make sure the person or organisation you are nominating knows about and agrees to the nomination.

You may nominate someone for one award category at a time. If you wish to nominate different people or organisations, please complete a separate form for each nomination.

Award values statement

Award recipients must demonstrate integrity, respect, service and conduct consistent with the values of the Parkinson’s community.

The judging panel may decide not to present an award where no nomination sufficiently meets the award criteria and required standards of character and conduct.

Before you begin

The form should take approximately 10–15 minutes to complete.

You will be asked to explain why the nominee deserves recognition and provide examples of their contribution or impact. Supporting documents are optional.

Please do not upload medical records or include detailed private health information.

Recognising an organisation, volunteer, advocate or community initiative making an outstanding contribution to the Parkinson’s community.
Recognising a person living with Parkinson’s or a carer whose courage, compassion, resilience, selflessness and integrity inspire others.

About the Nominee

Please do not provide details about the nominee’s diagnosis, medication, treatment or medical history. The judging panel only needs sufficient information to understand the nominee’s contribution and the reason for the nomination.

Tell Us Why You Are Nominating Them

Describe the nominee’s contribution, the difference they have made and why their work or example deserves recognition. Please provide specific examples wherever possible.
Consider the quality of their work, person-centred practice, professional leadership, collaboration and commitment to improving outcomes.
Evidence may include research findings, service results, community feedback, adoption, improved access or other measurable outcomes.

Character and Award Suitability

The Parkinson’s Industry Awards are public honours. The judging panel must be satisfied that recipients demonstrate appropriate character, integrity and conduct.

Review and Submit

 The Parkinson’s Centre collects the personal information provided in this form to administer the 2026 Parkinson’s Industry Awards, assess nominations, contact nominators and nominees, undertake appropriate checks and coordinate the Awards presentation.

Information may be shared with authorised Awards administrators and members of the independent judging panel. It will not be used for unrelated marketing without separate consent.

Shortlisted nominees will be contacted before their name, photograph, story or nomination information is published.

If the requested information is not provided, we may be unable to assess the nomination.
